At Vocentri, our Life Care Planning process provides a comprehensive, evidence-based projection of an individual’s long-term medical, rehabilitation, and support needs. Each plan is built on a foundation of professional expertise, collaboration with healthcare providers, and a clear understanding of the person’s goals for independence and quality of life.

Our Process

The process begins with an in-depth interview with the evaluee and a thorough review of relevant medical, vocational, and rehabilitation records. When appropriate, we conduct live video assessments to observe functional abilities, daily routines, and environmental factors in real time. Input from family members, caregivers, and interdisciplinary team members is incorporated to ensure a holistic understanding of the individual’s strengths, needs, and daily functioning.

Throughout the process, Vocentri consultants collaborate with treating physicians and other healthcare professionals to document their recommendations and medical opinions.
